In recent years, patients discharged to post-acute facilities have become medically more complex. Improving transitions from hospitals to post-acute facilities is becoming increasingly more important and more challenging. The need to focus on clinical quality, patient safety, and cost of care has increased but the silos that exist between hospitals and post-acute care facilities has widened. These silos result in medication errors, communication gaps, delayed treatments, poor patient experience, and clinician frustration. When TeamHealth received a request to improve transitions of care for patients being discharged from a Florida hospital to surrounding post-acute facilities, it presented an ideal opportunity to implement the TeamHealth Acute to Post-Acute Program (ATP). Hip fractures are some of the most common fractures seen in the emergency department (ED), and the number of hip fractures is only estimated to increase – almost doubling by 2050. Fall-related hip fractures among older patient populations are particularly common, with about 300,000 fall-related cases each year. Given the complexity of these cases, driven in part by co-morbidities in an older patient population, surgical complications can be serious. Blood-related complications are one of the most common. In June of 2024, TeamHealth and Los Robles Regional Medical Center embarked on a partnership to bring exceptional healthcare to the Thousand Oaks, California, community. The Los Robles emergency department was under construction, and the team only had 30 days to complete the transition, which normally is accomplished in 90 to 120 days. Previously, the emergency department had suffered from poor community perception, and the department was not performing to its fullest potential. The Los Robles leadership sought to drive change and better serve their community, so they partnered with TeamHealth.

“One of the biggest opportunities we had was a culture change in the emergency department,” says Scott Scherr, MD, FACEP, Regional Medical Director. “Everybody was working in silos, but we tore down that wall to where everybody’s voice matters and everybody is held accountable to providing excellent patient care.”

Surgical procedures tend to be stressful events for patients, so while an often-overlooked focus area for patient experience, the perioperative space provides many opportunities to enhance patient experience and outcomes.  Fisher-Titus Medical Center in Norwalk, Ohio, partners with TeamHealth for anesthesiology services and has benefited from our proprietary patient experience program, APEX.

TeamHealth’s Performance and Innovations Consultants (PIC) team brought the APEX program to Fisher-Titus to empower the anesthesiology team to improve surgical patients’ experiences. Current transfusion practice has a significant impact on hospital resources, including operational budgets. Experience and data have demonstrated that bloodless patients, treated by protocol and without transfusions, have done as well or better than those who are routinely transfused. This care has evolved to include all patients under the PBM concept and provide a single standard of care. Recently, an expert, interdisciplinary group produced a global definition of patient blood management: “a patient-centered, systematic, evidence-based approach to improve patient outcomes by managing and preserving a patient’s own blood, while promoting patient safety and empowerment.” TeamHealth’s Anesthesiology Performance and Innovations team partnered with a 199-bed Florida acute care facility to implement patient blood management (PBM) in the cardiac surgery service line.

Impact of Patient Blood Management after successful implementation

The goal of PBM is to improve patient outcomes through anemia management, optimized coagulation, blood conservation and patient-centered decision-making. These improvements are made possible by the close collaboration and support across disciplines. Consequently, facilities realize the benefits of improved clinical outcomes.
